Una preoccupazione per il crescente uso di farmaci da prescrizione durante la gravidanza è il rischio teratogeno. Questo rischio non è determinato per la maggior parte dei farmaci approvati negli Stati Uniti (USA) dal 2000 al 2010. L’acne e la psoriasi sono malattie croniche che si verificano tipicamente durante l’età fertile e poiché i retinoidi topici sono raccomandati sia per il trattamento dell’acne che per la psoriasi, è è possibile che le donne siano esposte a un retinoide topico durante la gravidanza. Gli studi di farmacocinetica mostrano un’esposizione sistemica relativamente bassa ai retinoidi topici, ma i livelli di esposizione che potrebbero portare a teratogenicità nell’uomo non sono noti. Il tazarotene, un retinoide topico, è stato approvato dalla Food and Drug Administration (FDA) statunitense sia per l’acne che per la psoriasi utilizzando i dati di farmacocinetica degli studi sulla psoriasi, che hanno stimato i dati basati sull’uso del tazarotene fino al 20% della superficie corporea.
Pertanto, sia in base all’etichettatura della gravidanza precedente che in quella attuale della FDA, il tazarotene non è raccomandato per l’uso durante la gravidanza. L’obiettivo di questa revisione della letteratura era fornire un contesto storico per la regola di etichettatura della gravidanza per il tazarotene rispetto ad altri retinoidi approvati e raccogliere i dati disponibili sugli esiti della gravidanza correlati a tazarotene e retinoidi. Sebbene vi siano segnalazioni di casi di esposizione topica alla tretinoina e all’adapalene in utero, non è chiaro se entrambe abbiano influenzato lo sviluppo fetale. In termini di tazarotene topico, ci sono attualmente dati limitati sugli esiti della gravidanza dopo l’esposizione in utero. Sono necessari ulteriori case report e studi sui risultati per esplorare ulteriormente la sicurezza del tazarotene topico in gravidanza.
La chemerina è indotta nella steatosi epatica non alcolica e nel carcinoma epatocellulare correlato all’epatite B
Chemerin è protettivo nei modelli sperimentali di carcinoma epatocellulare (HCC). Degno di nota, l’mRNA e le proteine della chemerina sono stati ridotti nei tessuti HCC di pazienti asiatici con eziologia per lo più della malattia da epatite B. L’attuale studio ha tuttavia dimostrato che la proteina chemerina è stata indotta nei tessuti tumorali di pazienti europei con HCC con steatosi epatica non alcolica (NAFLD) e pazienti con eziologia della malattia poco chiara. Una regolazione simile è stata osservata nel virus dell’epatite B (HBV), ma non nel virus dell’epatite C (HCV), HCC correlato. L’apparente discrepanza tra la regolazione della chemerina nell’HBV-HCC ottenuta dal nostro studio e rapporti recenti ci ha portato ad utilizzare gli anticorpi chemerina applicati nei saggi precedenti. Questi anticorpi non sono stati in grado di rilevare ugualmente diverse isoforme di chemerina, che erano sovraespresse nelle cellule HepG2.
Tuttavia, l’uso di tutti gli anticorpi ha confermato la presenza di una maggiore proteina chemerina nell’HCC. La proteina chemerina era bassa nelle cellule Huh7 e PLC / PRF / 5, mentre le cellule HepG2 e Hep3B avevano una proteina chemerina simile agli epatociti umani primari. Inoltre, gli effetti antitumorali dei retinoidi nelle linee cellulari degli epatociti non includevano la sovraregolazione della chemerina, che è stata inizialmente scoperta come proteina indotta dal tazarotene nella pelle. Infine, i livelli di proteine del recettore delle chemerine, il recettore 1 simile alle chemochine (CMKLR1), sono diminuiti nei tessuti non virali e tendevano ad essere inferiori nei tessuti HBV-HCC, suggerendo una ridotta attività delle chemerine nei tumori. Per riassumere, il nostro lavoro ha mostrato una regolazione opposta di chemerina e CMKLR1 in NAFLD e HCC associato a HBV. Nell’HCV-HCC né la chemerina né il suo recettore sono stati modificati nei tessuti tumorali. I risultati attuali non supportano un ruolo critico dei livelli di proteine totali delle chemerine nell’HCC di eziologia non virale e virale. Di conseguenza, la proteina chemerina localizzata nel tumore non era associata alla classificazione delle metastasi del nodo tumorale.
Test di fotostabilità di un retinoide-tazarotene di terza generazione in presenza di assorbitori UV
L’esposizione di un farmaco all’irradiazione UV potrebbe influire sulle sue proprietà fisico-chimiche. Pertanto, il test di fotostabilità è essenziale per i farmaci somministrati per via topica. Il tazarotene, un retinoide di terza generazione selettivo per i recettori, è comunemente usato per curare l’acne vulgaris e la psoriasi. Nel presente studio è stata eseguita un’analisi approfondita della fotostabilità del tazarotene in soluzione etanolica in presenza di ossido di zinco e / o biossido di titanio e filtri UV di tipo benzofenone. Undici presunti prodotti sono stati derivati dalla degradazione fotocatalitica del tazarotene utilizzando la cromatografia liquida ad alte prestazioni-spettrometria di massa tandem e sono stati proposti percorsi di trasformazione.

Il processo di degradazione ha interessato principalmente la porzione 4,4-dimetil-3,4-diidro-2H-tiopirano. I frammenti più suscettibili all’ossidazione erano i gruppi metilici e l’atomo di zolfo. Inoltre, in presenza di sulisobenzone, sotto irradiazione UV, il tazarotene è stato sottoposto ad un processo di degradazione, che ha portato a due prodotti di fotodecomposizione. Studi in silico eseguiti da OSIRIS Property Explorer hanno dimostrato che cinque dei prodotti di degradazione potrebbero essere dannosi in termini di effetti riproduttivi, che sono associati a 3,4-diidro-6-metil-2H-1-benzotiopirano 1,1-diossido, mentre uno di loro ha dimostrato una potenziale attività irritante. Le proprietà citotossiche dei prodotti di degradazione del tazarotene sono state valutate mediante dosaggio MTT su un pannello di cellule cancerose umane aderenti. L’inibizione della crescita dipendente dal tempo e dalla concentrazione è stata evidenziata nelle linee cellulari di cancro dell’ovaio (A2780) e della mammella (MDA-MB-231). La potenziale implicazione dei risultati della presente ricerca richiede ulteriori studi riguardanti principalmente la fotostabilità del tazarotene nelle formulazioni topiche.
