Scopo: descrivere la prima serie di sei pazienti giovani con melanoma uveale (UM) con isotretinoina orale e / o terapia topica con retinoidi prima della diagnosi.
Osservazioni: la serie di casi si basa su osservazioni cliniche presso il nostro centro di riferimento quaternario UM. Vengono segnalati sei casi di pazienti UM, di età compresa tra 16 e 44 anni. Tutti avevano usato un trattamento con retinoidi orale (isotretinoina) e / o topico (tretinoina o tazarotene) (3 mesi- ~ 10 anni) prima o al momento della diagnosi (3 su 6 casi). Tutti i pazienti presentavano disturbi oculari e l’insorgenza di alcuni sintomi corrispondeva al decorso dei retinoidi. Altri potenziali fattori di rischio o anamnesi rilevante includevano sfondo caucasico, distrofia conica e stato di fumatore attivo (caso 2), storia familiare di UM e gravidanza al momento della diagnosi (caso 3), fumo passato e possibile esposizione secondaria a Chernobyl da bambino ( Caso 5). Tutti i pazienti sono stati trattati con radioterapia a fascio di protoni e attualmente non hanno segni di malattia recidivante o metastatica.
Conclusioni e importanza: la terapia con retinoidi è stata collegata a vari effetti benigni e / o reversibili sull’occhio anteriore e posteriore, sebbene la fisiopatologia non sia ancora ben compresa. Il melanoma uveale (UM) è una rara diagnosi di cancro nei giovani adulti. Riportiamo qui la prima serie di casi di giovani pazienti di UM con una storia di uso di retinoidi e disturbi oculari. Non viene affermato alcun nesso causale e ulteriori studi epidemiologici e biologici sistematici sulla terapia con retinoidi e sull’impatto oculare possono fornire ulteriori dati rilevanti, in particolare nei pazienti giovani con melanoma oculare.
Aggiornamento teratogeno: uso topico e retinoidi di terza generazione
I primi retinoidi farmaceutici approvati dalla Food and Drug Administration degli Stati Uniti hanno ricevuto avvertimenti da scatola nera contro il loro uso in gravidanza a causa di potenziali effetti teratogeni. Questi retinoidi di prima e seconda generazione sono stati inizialmente formulati per il dosaggio orale e sono strutturalmente molto simili alla vitamina A, che ha effetti benefici sulla pelle e svolge un ruolo fondamentale nel guidare l’embriogenesi sana. Da allora, alcuni di questi retinoidi precoci sono stati riformulati per l’applicazione topica, il che ha ridotto il loro potenziale di esposizione sistemica. Inoltre, la progettazione razionale dei farmaci è stata applicata per creare i retinoidi di terza generazione di oggi (adapalene, tazarotene e bexarotene).
Questi composti includono anelli aromatici all’interno dei loro nuclei molecolari per fornire una rigidità strutturale che contrasta con la spina dorsale alifatica flessibile della vitamina A e delle precedenti generazioni di retinoidi, e quindi limita la loro attività fuori bersaglio. Come risultato di queste caratteristiche progettuali, il potenziale teratogeno negli animali dei retinoidi di terza generazione e in quelli riformulati per uso topico è generalmente inferiore a quello osservato con la somministrazione orale di generazioni precedenti di retinoidi. I dati epidemiologici disponibili, ma limitati, mostrano inoltre un potenziale teratogeno minimo o nullo associato all’uso nella vita reale di questi composti nell’uomo. Data la scarsità di dati epidemiologici disponibili in questo momento, tuttavia, si raccomanda di evitare l’uso di retinoidi topici durante la gravidanza. Tuttavia, in circostanze in cui può verificarsi un’esposizione involontaria in gravidanza, i dati disponibili forniscono una certa rassicurazione sul fatto che gli esiti avversi della gravidanza sono improbabili.
Chemerin come un driver di ipertensione: una considerazione
La proteina chemerina (gene indotto dal tazarotene, TIG2; RARRES2) è un’adipochina relativamente nuova. Molti studi supportano che i livelli di chemerina circolanti si associano fortemente e positivamente con BMI, grasso viscerale e pressione sanguigna. Qui, ci concentriamo sulla relazione specifica tra chemerina e pressione sanguigna con l’obiettivo di capire se e come la chemerina guida i cambiamenti (patologici) della pressione sanguigna in modo tale da poter essere interferiti terapeuticamente. Analizziamo la biosintesi della chemerina e come gli attuali farmaci antipertensivi modificano il metabolismo delle chemerine. Segue una rassegna di ciò che è noto su dove la chemerina è sintetizzata nel corpo e cosa possono fare la chemerina ei suoi recettori per la funzione fisiologica degli organi importanti per la determinazione della pressione sanguigna (ad es. Cervello, cuore, reni, vasi sanguigni, surrene e sistema nervoso simpatico).
Sintetizziamo dalla letteratura la nostra migliore comprensione dei meccanismi con cui la chemerina modifica la pressione sanguigna, con la consapevolezza che i livelli plasmatici / sierici di chemerina possono essere limitati nella loro rilevanza patologica. Questa revisione rivela diverse lacune nella nostra conoscenza della biologia delle chemerine che potrebbero essere colmate dal lavoro collettivo di chimici delle proteine, biologi, farmacologi e medici. Il tazarotene è un profarmaco che appartiene alla classe acetilenica dei retinoidi. Il farmaco è stato sottoposto a stress test idrolitico, ossidativo e fotolitico per stabilire la sua chimica di degradazione completa. Il farmaco si è dimostrato instabile in condizioni idrolitiche acide e basiche, producendo acido tazarotenico, che è un noto prodotto di degradazione principale (DP) e un metabolita attivo.
